Patient Identifier

This form hosts patient identifying information, including patients’ date of birth, and site-specific identifiers. When exporting patient data, identifiable information will need to be excluded from data export (see data export instructions).

Note: ONLY collect patient information when there is explicit IRB, institution, or direct consent approval.

This will allow the storage, sharing, and analysis of completely de-identified data sets outside this registry platform, which preserves patients’ privacy and enhances data security.
