Index Clinicopathological Diagnosis

This form captures the information about the leading diagnosis of a patient’s renal disease.


The date this diagnosis was made marks the INDEX DATE. This is the date to which all other dates relate and are linked.

  • Please note that this form is only for the patient's first case of renal disease diagnosis. All following renal disease diagnoses should be recorded as Subsequent Clinicopathological Diagnosis.

  • If the options provided do not capture the patient’s index renal diagnosis, please select “Other: Disease not listed in this section” and specify in the field that appears adjacent.

  • If the index clinicopathological diagnosis date is unavailable, use July 1 with the approximate year of diagnosis. If an exact year is not available, an approximation is acceptable. If any approximations are used, please make a note in the comments section.

  • Once the date is entered, please click on the calculator to calculate the age at the time of the INDEX diagnosis date.


All dates will be removed to create a completely deidentified dataset for exporting, downloading, and analyzing patients’ data.


The index date is set as time zero, and all other date elements are calculated as days or years relative to the index date (e.g., instead of the date of birth, the deidentified data set will include age at the time of the index date. Or instead of the date when the patient started dialysis, the data set will include the number of days from the index date when dialysis was initiated).
