Comorbidities

This form captures information about the patient’s major comorbidities. Major comorbidities are defined as diseases that significantly impact a patient’s life and well-being or may have a possible relationship to the patient's kidney disease. They should be listed as parts of the patient's problem list or past medical history in their health records. The comorbidities of interest are categorized into:


  • Cardiovascular comorbidities

  • Metabolic comorbidities

  • Infectious comorbidities

  • Autoimmune disorders

  • Malignancies

  • Other major comorbidities


Please select every comorbidity that applies to the patient by entering the date of diagnosis for that comorbidity under its name.

 

  • If the comorbidity date is unavailable, use July 1 with the approximate year of diagnosis. If an exact year is not available, an approximation is acceptable. If any approximations are used, please make a note in the comments section.

  • Once the date is entered, please click on the calculator to calculate the age at the time of the INDEX diagnosis date.



This form includes 20 pre-selected distinct comorbidities of interest. , For any other comorbidity the patient may have that does not fit in the pre-defined comorbidities of interest, please enter the information in the Other Comorbidity a table for autoimmune disorders, a table for any history of malignancy, and a table for any other comorbidity the patient may have that does not fit in the above categories (such as sickle cell disease, monoclonal gammopathy of unknown significance, depression).
