Subsequent Clinicopathological Diagnosis

Often, the patient's diagnosis changes, or additional diagnoses are made (e.g., a first biopsy diagnosis of class III lupus nephritis and a subsequent biopsy showing class V lupus nephritis).


This form captures the information about any follow-up diagnosis made clinically or through histopathology.


If the options provided do not capture the patient’s index renal diagnosis, please select “Other: Disease not listed in this section” and specify in the field that appears adjacent.


If there is more than one subsequent diagnosis, click on the form index icon and add another Subsequent Clinicopathological Diagnosis under Log Forms.


  • If the date of subsequent clinicopathological diagnosis is unavailable, use July 1 with the approximate year of diagnosis. If an exact year is not available, an approximation is acceptable. If any approximations are used, please make a note in the comments section.

  • Once the date is entered, please click on the calculator to calculate the age at the time of the INDEX diagnosis date.
