Genetic Testing

This form captures information about any genetic testing the patient may have undergone.

• Select "No" if no genetic testing was conducted.

• If genetic testing was conducted, choose the appropriate option:

  • "Yes, but the results are unknown/not available,"

  • "Yes, but no genetic cause of renal disease was identified," or

  • "Yes, results available."

If a genetic test is done and results are available, enter the results into the form and add a new row for each gene tested that was described in the genetic test results.
